- Weather changes: Drastic changes in the weather may affect the circulation in the body, causing facial skin to flush. Burst blood vessels can occur, resulting in facial spider veins.
- Changes in pressure: Sudden, extreme changes in pressure may cause small, broken blood vessels to appear. An exceptionally hard sneeze or a bout of vomiting can cause this change.
- Pregnancy: Hormonal changes during pregnancy may also cause broken blood vessels. Pregnancy-related spider veins usually disappear after giving birth.
- Environmental or chemical irritants: Exposure to certain chemicals or environmental pollutants may damage the skin and make blood vessels more visible.

We classify facial veins in terms of their size and depth, and where they are found, i.e., either within or under the dermis, and the anatomical area of the face affected. Fortunately, there is a good correlation between the anatomical location and the type of vein problem. Therefore, when we describe veins by the anatomical area on the face, it usually indicates what sort of vein is likely to be found.
Telangiectasia (‘spider’ or ‘thread’ veins)
Spider veins are broken blood vessels. Very superficial and very fine veins characterize facial telangiectasia. If bright red, the blood is usually in small arterioles that lie before the capillaries. Blue or purple blood usually lies in the veins after the capillary network.
Subdermal Veins
When veins are both large and deep enough, they can cause the skin to bulge above them, giving the appearance of "varicose veins." Of course, a facial "varicose vein" is not the same as one in the legs. Varicosity symptoms occur in the legs because the valves fail, allowing blood to accumulate due to gravity. In facial veins, blood flows with gravity back toward the heart. However, these veins can still dilate, resulting in cosmetically unsightly bulges, particularly when smiling, talking, straining or leaning forward.
Non-Invasive Treatments for Visible Facial Veins
Laser Technology and Advanced Options
Laser technology has revolutionized the treatment of visible facial veins, offering effective results with minimal discomfort. Vascular lasers are commonly used for conditions such as facial telangiectasia, delivering long-lasting results for unsightly veins. These devices work by emitting laser energy that heats the blood within the vein, causing it to collapse and fade over time. This minimally invasive procedure is suitable for a wide range of skin types and provides noticeable improvements in the appearance of facial veins.
Intense Pulsed Light (IPL) Treatment
Intense pulsed light, or IPL treatment, is another non-invasive option for treating visible blood vessels. Unlike lasers, IPL uses broad-spectrum light to target the skin’s surface and surrounding tissue. This treatment is particularly effective for improving the appearance of facial veins and addressing facial redness caused by dilated blood vessels.
IPL treatment produces very concentrated, well-controlled bursts of white light in pulses to target veins. It also includes some infrared and ultraviolet wavelengths, ranging from about 400nm through to 1200nm. In IPL therapy, your provider can select a variable number of pulses, each of a defined length, with a specified rest period between each pulse. We can tailor treatments to various skin types and tones, making it a suitable choice for individuals with darker skin tones or sensitive skin. By directing light energy toward problem areas, IPL reduces visible facial veins while promoting an even-toned skin appearance. Patients often experience minimal discomfort during the procedure, and the treated area requires little downtime, allowing for a quick return to normal activities.
The intensity of the white light causes it to heat tissue. Different handpieces for intense pulsed light therapy place various filters in front of the beam before it hits the skin. As the absorption peaks for oxyhemoglobin (predominantly found in red veins) and deoxygenated hemoglobin (predominantly found in blue veins) are 418nm and 542nm, respectively, this setting is ideal for treating facial spider veins. IPL handpieces often have a wide ‘footprint,’ meaning that relatively large areas can be treated with each shot fired, making it ideal for large areas involving diffuse spider veins or rosacea.
Transdermal Laser
LASER is an acronym for light amplification by stimulated emission of radiation. Although some lasers do produce light, others produce microwaves invisible to the naked eye. Unlike IPL, which uses white light and then selects bands of wavelengths with filters, intense laser light produces a beam of a single wavelength.
Because a laser emits a precise wavelength, appropriate laser therapy can target specific pigments, such as oxyhemoglobin or hemoglobin. When a laser treatment targets a pigment or chemical, it is called a ‘chromophore’. Chromophores absorb or reflect various wavelengths of light. When it absorbs sufficient energy, it can be disrupted or can heat up. Therefore, for vascular lesions like thread veins on the face, we use a wavelength readily absorbed by oxyhemoglobin or deoxygenated hemoglobin. Such lasers include the KTP vascular laser (532nm – green light) and the ND:YAG (1064nm – microwave) lasers, although other lasers can also be used. The KTP does not penetrate very deeply and is used for very superficial bright red or bright blue veins. It heats the hemoglobin, causing the heat to be passed into the vein wall and destroying the vein. The ND:YAG, which has a wavelength exactly twice that of the KTP, also interacts with hemoglobin, causing it to heat. However, this longer wavelength penetrates deeper into the skin, allowing treatment of larger green veins deep under the skin, effectively reaching a depth of 1.5 mm.
Laser therapy can accommodate many different factors. The "spot size" can vary, allowing your provider to target very small or extensive areas. We can change the power used in an area and vary the pulse length. Pulses usually last milliseconds. With access to lasers of different wavelengths, spot sizes, and powers, we can successfully treat small blood vessels, such as spider veins and reticular veins, in the majority of cases.
Invasive and Minimally Invasive Treatment Methods
Spider Vein and Thread Vein Treatments
For larger veins and deeper veins, invasive or minimally invasive treatments may be necessary, often involving multiple treatment sessions to address the number of veins effectively. These procedures improve blood flow in the treated areas by closing off visible veins, leading to a more even complexion.
Thread veins, which are smaller and closer to the skin's surface, are typically treated with precise techniques that minimize impact on surrounding areas. Recovery from these treatments is usually quick, allowing patients to resume normal activities shortly after their session. These methods are particularly effective in reducing the appearance of broken blood vessels and other vein conditions.
Sclerotherapy
The use of sclerotherapy in facial veins is controversial. Sclerotherapy is used to treat small veins on the legs and body, and, when made into a foam, has been used to treat larger veins. Sclerotherapy for facial veins is off-label and should be performed only by experienced medical practitioners. Experts have pointed out that sclerotherapy can be used on larger blue spider veins but may produce complications from inadvertent injection into arterioles. This can lead to skin damage (necrosis) or, very rarely, blindness in one eye. The sclerosing agent, once injected into the blood vessels, will eventually close the veins, and they will disappear in a few weeks. Patients generally need more than one session to treat the veins effectively and safely. The higher the volume of sclerosant injected, the greater the risk of complications, so the amount of sclerosing agent injected into blood vessels should be limited to small doses.
Phlebectomy
Phlebectomy of facial veins is technically challenging and requires considerable practice. Incisions are made using the smallest possible blade, aligned with where skin creases will develop. Of course, if there are already skin creases, incisions should be made within them. Phlebectomy hooks can be used, but it should be noted that these veins are tiny and difficult to remove. With experience, patients can get good results, but only experienced medical professionals should perform this procedure.
