SERVICES AND FEES
Both Dr. Sidhu and Dr. Kaur are a licensed U.S. Civil Surgeons who are authorized by the Bureau of Citizenship and Immigration Services to conduct Immigration Physical Examinations for the Department of Homeland Security.
For this service, our doctors will be required to complete the I-693 form. They will review your detailed medical history and perform a physical examination. If you need any additional vaccinations or blood work we will arrange for these services. Only the physician can determine which tests you need.
WHAT TO EXPECT
When you schedule an appointment with one of our doctors for the USCIS immigration examination you will be charged $350 which will include the exam, filling out the I-693 form and the associated administrative work.
You may also need additional testing and vaccinations. These two services can be covered through your insurance if you have one. If you don’t have insurance information on associated costs will be provided.
WHAT TO BRING
- Valid government issued picture ID such as a driver license or passport
- Vaccination and other relevant medical records if you have any in English.
- Bring a list of any medication you are taking. If you are being treated for a chronic medical condition or taking medication on a regular basis, be ready to explain your condition(s).
AFTER THE EXAM
After the immigration physical examination is completed, our physician will certify the I-693 form. This paperwork will be given to you in a sealed envelope. DO NOT OPEN THE SEALED ENVELOPE. Turn in the envelope with your immigration application. A copy of the enclosed forms will also be provided for you to keep. The examination is valid for two years after the immigration physical.
